P-EBT Card Ohio Activation Steps:
- Locate the Phone Number: Find the customer service phone number printed on the back of your P-EBT card.
- Call and Follow the Prompts: Dial the phone number and listen carefully to the automated instructions.
- Enter Required Information: When prompted, enter the following details:
- P-EBT card number
- Child’s date of birth (YYYY format, e.g., 2010 for a birth year of 2010) – Important Note: Be sure to enter the year of birth, not the last four digits of your child’s Social Security number.
- Your zip code
- Create a PIN: You’ll be prompted to create a four-digit PIN for future transactions. Choose a memorable PIN and keep it confidential.
Congratulations! Your P-EBT card is now activated and ready to use for purchasing eligible food items at authorized retailers that accept SNAP/EBT cards.
Additional Information:
- While the card is issued in your child’s name, they don’t need to be present for purchases.
